Çocuk nefrolojisi; bebeklik, çocukluk ve ergenlik döneminde böbrek ve idrar yollarını ilgilendiren hastalıkların tanı, takip ve tedavisiyle ilgilenen pediatrik yan daldır. Çocukluk çağında böbrek hastalıkları, erişkinlerden farklı seyredebilir ve büyüme-gelişme sürecini doğrudan etkileyebilir. Bu nedenle çocuklara özgü, multidisipliner ve uzun dönemli bir takip yaklaşımı gerektirir.
Böbrekler; vücuttaki sıvı-elektrolit dengesini sağlamak, zararlı maddeleri süzmek, kan basıncını düzenlemek ve kemik gelişimini desteklemek gibi hayati görevler üstlenir. Çocukluk döneminde ortaya çıkan böbrek hastalıkları erken dönemde fark edilmezse, ilerleyen yaşlarda kalıcı böbrek hasarına yol açabilir. Bu nedenle düzenli takip, doğru tanı ve zamanında tedavi büyük önem taşır.
Çocuk nefrolojisi birimi başlıca şu durumların değerlendirilmesi ve takibini yapar:
İdrar yolu enfeksiyonları ve tekrarlayan enfeksiyonlar
Doğumsal böbrek ve idrar yolu anomalileri
İdrarda kan (hematüri) veya protein kaçağı (proteinüri)
Nefrotik sendrom
Glomerülonefritler
Böbrek taşları
Yüksek tansiyon (çocukluk çağı hipertansiyonu)
Akut ve kronik böbrek yetmezliği
Gece alt ıslatma (altta yatan böbrek nedenleri açısından değerlendirme)
Çocuk nefrolojisi değerlendirmelerinde, çocuğun yaşı, büyüme-gelişme durumu ve klinik bulgular birlikte ele alınır. Gerekli görülen durumlarda laboratuvar testleri, idrar incelemeleri, görüntüleme yöntemleri ve düzenli takip planları oluşturulur. Amaç yalnızca mevcut hastalığı tedavi etmek değil, çocuğun sağlıklı gelişimini uzun vadede korumaktır.
Aşağıdaki durumlarda çocuk nefrolojisi değerlendirmesi önerilir:
Sık tekrarlayan idrar yolu enfeksiyonları
İdrarda köpüklenme, kan görülmesi veya renk değişikliği
Nedeni açıklanamayan ödem (şişlik)
Yüksek tansiyon saptanması
Böbrekle ilgili doğumsal bir sorun tanısı
Uzun süren veya açıklanamayan büyüme-gelişme geriliği
Çocuk nefrolojisi, gerektiğinde çocuk sağlığı ve hastalıkları, çocuk cerrahisi, radyoloji ve diyetetik gibi branşlarla birlikte çalışır. Bu bütüncül yaklaşım sayesinde tanı ve tedavi süreçleri çocuğun genel sağlığı gözetilerek planlanır.
